False PHD is a passive item added in The Binding of Isaac: Repentance that fundamentally alters how pills work by converting positive effects into negative ones, but offering high-utility combat benefits in return. The Stat-Down Tradeoff: Converts all positive stat pills into their negative counterparts. However, consuming any “Stat Down” pill permanently grants Isaac +0.6 Damage (+1.2 Damage for Horse Pills). This bonus applies retroactively to stat-down pills taken earlier in the run.

If you manage to collect both PHD and False PHD in the same run, they do not completely cancel each other out. Instead, they create a unique synergy:
The pill pool returns to normal, meaning you can find both good and bad pills again.
When you encounter and consume a bad pill, you still retain the False PHD benefits (the permanent damage ups or the black heart drops).
This combination effectively eliminates the primary downside of False PHD while keeping its offensive and defensive scaling intact.
